1. Henry OT Services, Inc. accepts orders on a Pre-Paid basis only, (Payment can include check or money order sent in with order form. No checks accepted from banks outside of the USA. Orders accepted online with valid MasterCard/Visa or Discover)

Purchase Orders are accepted in writing from USA schools/organizations with prior approval from our office! Purchase Orders may be mailed to the address listed below. Purchase orders must be signed and must include a contact phone number.

2. Payment: Make checks payable to Henry OT Services. MasterCard/Visa and Discover are accepted online-make sure all 16 digits of the account number are included along with the expiration date and verification number, the three digit number on the signature panel. All payments must be made in US funds. No checks accepted from banks outside of the USA.

International orders: All foreign orders must include valid MasterCard or Visa number so that additional shipping charges can be covered.

3. Minimum order requirements: $50.00 for Purchase Orders

4. Shipping: For domestic orders shipped via Standard Mail or UPS Ground: Continental United States, Alaska, Hawaii, Puerto Rico

Orders to Canada- Add 20% of Subtotal (minimum $12.00) Prices in U.S. dollars and do not include VAT, customs, duties or taxes

5. Delivery: Orders are shipped via UPS ground or US Mail. We attempt to process and ship all orders within 2-3 BUSINESS days. If express delivery or service is desired, do not submit your order on-line. Please call your express orders in to 623-882-8812 before 12:00pm MST, Monday - Thursday.

8. Sales Tax: Arizona residents must include 8.5% sales tax.

9. Returns are accepted within 30 days for a full refund as long as the item(s) are in new sellable condition. Exceptions: Due to copyright law, returns or refunds cannot be accepted for any recordings. This includes Video Tapes, Audio Tapes, and Compact Discs. If an item is defective we will gladly exchange for a like item. Exchanges for any other reason cannot be accepted.
